To use the new theme in your Gmail inbox, go to Settings > Themes > Save. You can change your theme as frequently as you want.

Creating a Gmail Theme from a Personal Photograph
Upload a photo to your free Google Photos storage to use as your Gmail background theme if you already have one on your computer. Learn how to get started with Google Photos if you haven't already.

At the bottom of the Choose a theme window, click My Photos. My Photos will be picked in the Select your background image window.
